The purpose of this position is to provide discrete oversight and supervision by coordinating personal care for individuals and providing hands-on care in homes. Duties for this position are, but not limited to, assist individuals with housekeeping tasks, provide accurate and timely documentation of tasks rendered for the customer, provide timely and complete assignment sheets and progress notes to the agency, and assist with personal hygiene tasks (including but not limited to: dressing, feeding, nutrition, and environmental support functions, bathing, grooming, toileting, walking, transfer, skin care, turning and positioning, and other assistance as needed).
Location- Herkimer and Oneida County
Requirements- High School Diploma or GED. Minimum of six months’ experience working in a home care setting within the current year. Reliable transportation for daily travel, New York State Driver License and acceptable driving record are required. Ability to lift a minimum of 60 lbs. Must meet the background and medical requirements of the program prior to hire.
Knowledge and Skills- Knowledge of Traumatic Brain Injury, Elderly and Aging, cultural diversity, and disability issues. Ability to effectively communicate changes in the consumer’s condition to immediate supervisor as well as ability to identify care and quality concerns.
Hourly Wage- $12.36/hr., $12.86/hr. after six (6) months of successful employment
