Non-Exempt Salary: $37,856
35 hour work week
Job Duties:
The successful candidate will be expected to:
- Meet with individuals who desire to transition from an institution to living in the community; identify what services are needed and assist with providing those services.
- Develop a comprehensive, person-centered transition plan.
- Identify and remove barriers for individuals transitioning to living in the community.
Knowledge, Skills & Abilities:
- Experience working with individuals with disabilities, either professionally or personally.
- Strong advocacy, networking, and organizational skills.
- Knowledge of community-based services, nursing home transition process and the Americans with Disabilities Act.
Education:
- An Associates Degree is preferred; a High School Diploma or equivalent is required.
- Experience may be considered in leu of higher education.
