Youth Advocate - Case Manager for At-Risk Youth

Annual Non-Exempt Salary: $37,849.50
Job Duties:
The Case Manager will serve youth with disabilities who may be at risk due to any of the following: behavioral issues, a lack of community-based support services, lack of achievement in school or lack of social skills.
The successful candidate will be expected to:
- Conduct a youth risk assessment to determine their potential for crisis.
- Complete Support Service Plan to support identified needs.
- Maintain regular contact with participants and their family members to monitor progress. This includes visits at mutually agreed upon locations (including the home).
- Maintain and/or update case files.
- Provide information and referral services.
Knowledge, Skills & Abilities:
- Experience working with youth and/or individuals with disabilities, either professionally or personally.
- Strong advocacy, networking, and organizational skills.
- Computer skills, including outlook and Microsoft programs.
- Knowledge of community-based services.
- Excellent customer service skills.
Education:
- An Associates Degree is preferred; a High School Diploma or equivalent is required.
- Experience may be considered in leu of higher education.
