Description

Looking for a Community Habilitation Worker to provide assistance to a Self-Direction Participant to live independently in an apartment and access the local community. Staff will assist the individual with housekeeping and other activities to include cleaning, organizing, general household maintenance, and shopping. Staff will assist with Self Direction paperwork/documentation. Staff will assist the individual with accessing gyms to exercise at and using equipment. Staff will assist with activities in the community to include: attending public events, shopping, banking, going to the movies, going to restaurants, and more. Looking for a Community Habilitation staff to work a couple days a week for approximately 8 hours a week.
