Description
To provide direct support and teach skills to a male participant in their early 40s. Lives independently in an apartment. • Assist and teach skills including cooking, cleaning, shopping, money management, and safety. • Support individuals out in the community including travel training, community integration, and providing transportation. • Teach social skills and support individuals in making community connections. • Assist individuals in living a healthy and active lifestyle by providing support at a community gym. • Foster independence at home and in the community. • Other duties as described in their Community Habilitation Plan.
