Youth Advocate-Case Manager for At-Risk Youth

Description
Annual Non-Exempt Salary: $37, 856.35
35 hour work week
Job Duties:
The successful candidate will be expected to:
- Provide information and referral services.
- Complete Support Service Plan to support identified needs.
- Provide information and referral services.
- Conduct a youth risk assessment to determine their potential for crisis.
- Maintain regular contact with participants and their family members to monitor progress. This includes visits at mutually agree upon locations (including the home)
Knowledge, Skills & Abilities:
- Experience working with youth and/or individuals with disabilities, either professionally or personally.
- Strong advocacy, networking, and organizational skills.
- Knowledge of community-based services.
- Excellent customer service skills.
- Computer skills, including outlook and Microsoft programs.
Education:
An Associates Degree is preferred; a High School Diploma or equivalent is required.
Experience may be considered in lieu of higher education.
