Self-Direction is a program funded by the State and Medicaid that allows individuals the ability to live independently in their own homes and the ability to integrate into the community. Each participant as well as each budget are unique. Self-Direction is completely consumer driven. Participants and their Planning Team decide what services will best suit their needs.
The Case Coordinator will maintain a caseload of participants and administer their services in the Self-Direction Program. The successful candidate will be expected to:
- Maintain open communication with all parties on the assigned caseload, return calls and emails within 24 hours to participants, their workers, Start-Up/Support Brokers, Designated Representatives and/or internal staff using exceptional customer service;
- Maintain required Medicaid documentation associated with the provision of services for the assigned caseloads in a timely, accurate and legible manner;
- Review monthly budget reports for accuracy and submit monthly invoices and reimbursements for payment as needed; and
- Process weekly timesheets for support workers as needed.

Who may be considered:
A minimum of two (2) years of customer service experience are required along with one-year experience in a human services organization and in a position requiring independent-decision making.
Strong customer service skills, the ability to multi-task, experience in Microsoft Office, and the ability to work in a fast-paced environment are required.
An Associates Degree in Human Services, Secretarial Science or business is preferred; a High School Diploma or equivalent is required.
