Veteran Directed Care Person Centered Counselor

Starting salary: $44,000

Individuals who are Veterans or have experience working with Veterans are strongly encouraged to apply.

Job Duties:

The successful candidate will be expected to:

  • Meet with Veteran to develop a comprehensive person-centered spending plan.
  • Acknowledges receipt of referral form from VAMC within one business day.
  • Contacts the Veteran within two business days of receiving referral to schedule an intake home visit.
  • Conduct quarterly in-person visits to monitor well-being of Veteran and review spending plan.
  • Communicate effectively with Veteran and/or Authorized Representative and provide counseling if not adhering to spending plan.
  • Assess, identify, coordinate, and monitor supports and services on an ongoing basis.
  • Have a valid NYS driver license, have own/reliable transportation, be able to travel, and have a flexible schedule to accommodate Veteran's need.

Education:

  • Social Work degree preferred. Other relevant education in the Human Services field will be considered. Experience may be considered in lieu of education.

Knowledge, Skills & Abilities:

  • Must be proficient in Microsoft Office suite applications for preparation of documents, spreadsheets,  and data  management.
  • Sensitivity and awareness of issues related to Veterans. Ability to work with diverse population.
  • Community networking and collaboration, advocacy, willingness to work a flexible schedule.
  • Excellent customer service skills, strong verbal and written communication and time management skills.
  • Knowledge of human services, Independent Living philosophy and case management preferred.
