Salary Range:$18.00 To 18.00 Hourly

Job Title: Personal Assistant 

Status: Hourly, Non-Exempt

Reports To: Consumer/Designated Representative

Purpose:  To provide supports to Consumers as written within their Plan of Care and as directed by the Consumer/Designated Representative.

Duties and Responsibilities:

  • Follow the Plan of Care as written. Tasks may include dressing, grooming, hygiene, transfers and/or lifting, toileting and health maintenance, light housekeeping, and errands.  
  • Follow the schedule determined by the Consumer/Designated Representative.
  • Maintain open communication with the Consumer/Designated Representative.
  • Follow the Personal Assistant Agreement.

Work, Education, Knowledge, and Skill Requirements:

  • The requirements are based on the Consumer’s needs and set by the Consumer in accordance with his/her/their Plan of Care.

Other:

  • Lifting may be required and will depend on Consumer needs. Meet the requirements regarding immunizations, physical health assessment and the mandated training upon hire and annually thereafter. Personal Assistants must be 18 years of age or older.
