Job Duties
- Work as part of a team to provide case management to individuals receiving services in the community through OPWDD funding (Self Direction program).
- Support individuals with disabilities to utilize their services and funds to live in the community.
- Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations.
- Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence.
- Document all communications.
Knowledge, Skills & Abilities
- 2 years of Case Management experience required. Higher education will be considered in lieu of experience.
- Strong customer service and computer skills.
- Ability to work in a fast-paced environment.
- Knowledge of Microsoft Excel, Word, and Outlook.
- Excellent verbal and written communication skills.
- Strong organizational skills and attention to detail.
Education and Experience
- A Bachelor or Master of Social Work degree is preferred.
- A college degree in Human Services or related field will be considered.
- A high school diploma or equivalent is required.
- Experience providing OPWDD funded services or other case management services may be considered in lieu of higher education.
