Job Duties
The successful candidate will be expected to:
- Serve as the first point of contact for referrals, conducting screenings and eligibility assessments using standardized tools (e.g., Units US platform).
- Identify and address social needs such as housing, food, security, transportation, and emotional well-being.
- Maintain accurate documentation of all outreach, screenings, and referrals; reports trends to support continuous improvement.
- Facilitate access to appropriate community and health resources, ensuring timely outreach, referral and follow-up.
- Participate in reviews, trainings, and community events to strengthen coordination and professional growth.
- Assist with both internal and external audits.
Knowledge, Skills & Abilities
- Must be proficient in Microsoft Office suite applications for preparation of documents, spreadsheets and data management. Basic knowledge of human services, Medicaid, disability awareness, understanding of the Independent Living philosophy.
- Strong computer skills are required. Strong verbal and written communication, attention to detail, problem solving, excellent organization, time management, and ability to work with a diverse population of individuals is required. Must be able to work independently and initiate work in a fast-paced environment. Excellent Customer Service skills required. A Valid NYS Driver’s License is required.
Education and Experience
- A College Degree in health, social services, or a related field preferred; a High School Diploma or equivalent is required.
