- Purpose:
To engage, assess, assist, and monitor HC department of Social Services (HCDSS) applicants and participants with disabilities having work actively requirements.

Job Duties
The successful candidate will be expected to:
- Receive referrals of program participants through department of Social Services.
- Review historical documentation with each applicant/participant and request additional assessments/services as needed.
- Development of a plan for employment based upon the assessment in conjunction with the participant including addressing childcare, transportation, and other concerns and appropriate referrals as needed.
- Compile relevant data pertaining to level of education achieved, including post-secondary, work experience, and duration, current health status and work limitation, disabilities, and concerns related to current and past barrier to securing and maintaining employment.
- Monitor status and assure community linkage in line with the employment plan to assure movement toward goals.
Knowledge, Skills & Abilities
- Two years of vocational experience with individuals with disabilities is preferred. Minimum of two years working with individuals with disabilities.
- Familiarity with disability and social services systems. Experience with working as part of a team. Microsoft Office, Outlook, Window proficiency.
- Assessment, networking, knowledge of community resources, advocacy, strong oral and written communication, data entry and strong organizational skills.
Education and Experience
- Associate degree is preferred, but high school diploma/equivalency is acceptable, along with relevant work experience.
