Everyone has rights and choices. At RCIL, we believe that no matter how old you are or what your level of ability is, you have the right to live independently and make your own choices. 

Open Doors / Money Follows the Person

Want to live in your own home or apartment? It's your choice! The Open Doors / Money Follows the Person program provides transition specialists to help people move from long-term care facilities back into their homes and communities.

 

Participation in this program is 100% voluntary. Once the decision to participate has been made, one of our Transition Specialists can work with you and your family members to you determine what services you will need to live on your own, as safely and independently as possible. We will also work with you to help you figure out how to get these essentail services; such as:

  • affordable and accessible housing
  • the information and skills you'll need once you return home
  • any other additional services that you may benefit from (such as service coordination or case management).

Choosing not to participate in the Open Doors / Money Follows the Person program, or ending participation with the program, will not affect your discharge and transition to the community.


Eligibility Requirements:

  • Needs to have been living in a nursing facility, hospital, or Intermediate Care Facility/IID for more than 90 consecutive days (excluding Medicare covered rehabilitative care that is expected to be short-term in nature),
  • Needs to choose to move to a qualified community residence, such as A) A residence owned or leased by an individual or his/her family, B) A residence in the community in which no more than four unrelated individuals live, or C) An apartment with an individual lease with lockable access and egress, and which includes living, sleeping, bathing, and cooking areas over which the individual of his/her family has control, and
  • Must have received Medicaid inpatient services for at least one day prior to moving.

Open Doors / Money Follows the Person is a federal grant working towards the goal of increasing the use of home and community-based services (HCBS) and reducing the use of institutionally-based services.
