Case Manager for At-Risk Youth

Description

Starting Pay Rate: $17.95 per hour

Job Duties:

The Case Manager for At-Risk Youth will serve at-risk youth, with and without disabilities, and their families.

The successful candidate will be expected to:

  • Work with youth who are at-risk.
  • Complete intake assessments.
  • Provide information and referral services.
  • Conduct a youth risk assessment to determine their potential for crisis and involvement with the juvenile/criminal justice system.
  • Maintain regular contact with participants and their family members to monitor progress.

 

Knowledge, Skills & Abilities:

  • Experience working with individuals with disabilities, either professionally or personally.
  • Strong advocacy, networking, and organizational skills.
  • Knowledge of community-based services.
  • Knowledge of ADA, IDEA, 504, Social Security, ACCESS-VR, CBVH and the juvenile/criminal justice system is preferred.

Education:

  • An Associates Degree is preferred; a High School Diploma or equivalent is required.
  • Experience may be considered in leu of higher education.
