Consumer Directed Personal Assistance Program Coordinator
Do you want to support chronically ill or physically disabled individuals get the services they need to live at home? The Consumer Directed Personal Assistance Program (CDPAP) provides services to individuals that want to live at home, but need skilled nursing services and/or help performing everyday tasks. As a CDPAP Coordinator you will assist individuals to start their own program so individuals can hire, train and supervise their own Personal Assistant(s).
The successful candidate will be expected to:
• Provide initial orientation and ongoing support to participants of CDPAP;
• Meet with participants and their potential employees in the participant’s home to explain CDPAP;
• Monitor and document participant’s ability to manage their services and provide additional trainings as needed;
• Input and maintain participant information into the Medisked electronic database; and
• Learn and remain current on the regulations and policies of CDPAP.

Who may be considered:
Two (2) years working in the human services field or relatable work experience is required.
An Associate’s Degree in a related field is preferred. A High School Diploma or equivalent is required.
Travel is required for this position.
