Person-Centered Budget Coordinator (Oneida County)
Self-Direction is a program funded by the State and Medicaid that allows individuals with disabilities the ability to live independently in their own homes and the ability to integrate into the community. Each participant as well as each budget are unique. Self-Direction is completely consumer driven. Participants and their Planning Team decide what services will best suit their needs.
The Person-Centered Budget Coordinator will develop individualized budgets and service plan updates yearly. The successful candidate will be expected to:
- Facilitate Planning Team/Circle of Support meetings and record meetings;
- Develop and update comprehensive person-centered plans;
- Create and update individualized budgets and service plans;
- Identify and access possible community and natural supports to aid individualized budget and service plans; and
- Provide education and assist the participant in finding and hiring support staff.
- Travel extensively (mileage reimbursement)
Who may be considered:
Successful candidates are required to have knowledge of OPWDD rules and regulations and two (2) years of experience working with individuals who have disabilities. Attention to detail, willingness to work a flexible schedule, time management, documentation, organization and customer service skills. Experience with Self Direction budgets and habilitation planning are preferred.
An Associate’s Degree in Human Services is preferred. A High School Diploma or equivalent is required.
Mileage reimbursement of .50 per mile.
